Costs:  $25.00 for members
           $40.00 for new members (includes 1 yr. mem)

Bring: - Authentic Birth Certificate (no copies)
           - Check or Money-Order Payment (no cash)
           - Copy of Last Report Card (issued 2nd week of June)
           - Complete Medical Physical (use your own physician or
             wait and obtain physical on July 26)

NO CHILD WILL BE ALLOWED ON THE PRACTICE FIELD UNTIL COMPLETELY REGISTERED
WITH ALL PAPER WORK ON FILE.
